How do I change the premium on my policy?

Fill an endorsement form and indicate the new and the old premium or indicate in writing the new and the old premium.

How do I change the term on my policy?

Fill a Pinnacle Life endorsement form and indicate the new term or submit a letter indicating the new term

Note: the new term must not be less than seven (7) years.

How can I change retirement age?

Fill a Pinnacle endorsement form and indicate the new retirement age or submit a letter indicating the new retirement age

Note: the difference between the commencement date and the new retirement date must not be less than seven (7) years.

How do I add lives on my policy?

Fill a Donewell funeral endorsement form.
Client must indicate the name and date of birth of the new life E.g. father, mother, spouse, etc.
Client must also indicate the current state of health on the form.

How do I make a claim

By completing a claim form and submitting it to any of our offices.
For surrenders and maturities add policy document and a nationally recognized photo ID card.
For loans, withdrawals, refunds and reimbursement add only nationally recognized photo ID card.

The following documents must be submitted in the case of death claim:

  1. Death certificate
  2. Medical certificate of cause of death
  3. Police report, in the case of accidental death
  4. Policy Document
  5. Nationally recognized photo ID
